Clearway Clinic is committed to protecting your privacy. Throughout the site, the terms “we”, “us” and “our” refer to Clearway Clinic, Inc. We have designed our website to allow you to visit most areas without identifying yourself or supplying any personal information. We will only collect personal identification information from you if you voluntarily submit such information to us. We will never intentionally disclose any personal identification information about you as an individual user (such as, for example, your full name, street address, telephone number, or email address) to any third party without having received your explicit permission. We do not sell or otherwise share your data with third parties. Below are outlined the only cases in which your data can be shared with third parties. Though we make every effort to preserve user privacy, we may need to disclose personal information wherein we have a good-faith belief that such action is necessary to comply with a current judicial proceeding, a court order or legal process. We may share with others aggregated demographic information not linked to any personally identifiable.
To better protect your privacy, we provide this notice explaining our online information practices and the choices you can make about the way your information is collected and used. To make this notice easy to find, we make it available at the footer of every page and every place where personally identifiable information might be requested. This policy is designed to inform you of the types of information we collect, how we use that information, and the circumstances with which we can share it with non-affiliated third parties.
Note: “Protected Health Information” as defined by the Health Insurance Portability & Accountability Act and related regulations (collectively referred to as “HIPAA”), including information you provide to Clearway Clinic while receiving services as a patient of any of our facilities, is separate and subject to the Clearway Clinic Notice of Privacy Practices.

This website takes precautions to protect our users’ information. When users submit sensitive information via the website, we take great effort to ensure that your information is protected both online and offline. When our registration/order form asks users to enter sensitive information (such as credit card number, social security number, and/or Federal Tax ID), that information is encrypted and is protected with SSL encryption software. While on a secure page, such as our order form, the lock icon on the bottom of Web browsers such as Netscape Navigator and Microsoft Internet Explorer becomes locked, as opposed to un-locked, or open, when you are just ‘surfing’. While we use SSL encryption to protect sensitive information online, we also protect user-information off-line. All of our users’ information, not just the sensitive information mentioned above, is restricted in our offices. Only employees who need the information to perform a specific job (for example, our billing clerk or a customer service representative) are granted access to personally identifiable information. Our employees must use password-protected screen-savers when they leave their desk. When they return, they must re-enter their password to re-gain access to your information. Furthermore, ALL employees are kept up-to-date on our security and privacy practices. Every quarter, as well as any time new policies are added, our employees are notified and/or reminded about the importance we place on privacy, and what they can do to ensure our customers’ information is protected. Finally, the servers that we store personally identifiable information on are kept in a secure environment, behind a locked door or cage.
If you have any questions about the security at our website, you can send an email to erlina@clearwayclinic.com or jill@clearwayclinic.com.
If a user’s nonpublic personally identifiable information changes (such as your zip code), or if a user no longer desires our service, we will endeavor to provide a way to correct, update or remove that user’s personal data provided to us. This can usually be done by calling our headquarters at 508-438-0144 or emailing jill@clearwayclinic.com.
If you have any questions regarding this document, contact us at jill@clearwayclinic.com.
